Adult/Pediatric ADHD Initial Appointment
Initial assessment for ADHD. The assessment is done in the clinic and involves a number of activities and questionnaires using evidence based guidelines for initial screening for ADHD. Parents will be sent home with various assessment forms including teacher assessment forms for completion prior to treatment initiation. Our Nurse Practitioner follows the Canadian ADHD Resource Alliance (CADDRA) Guidelines.
Adult/Pediatric ADHD Appointment #2
This appointment is to review forms provided for take home completion; WEISS Screening Tool, SNAP-IV, CADDRA Teacher Assessment Form. Initial treatment for ADHD will be discussed using a collaborative approach between Nurse Practitioner, patient and parents. The CADDRA Patient ADHD Medication Form will be provided for home completion to monitor side effects of medication treatment. This form will be reviewed at the 3rd appointment.
Adult/Pediatric ADHD Appointment #3
This appointment is to determine a comprehensive treatment plan, including prescription medications, counseling, supplementation, and cognitive behavioral therapies that are safe and effective for the management of ADHD.
Adult/Pediatric ADHD Appointment #4
A follow-up in person every 4 weeks is expected to review effectiveness and tolerance to current therapies, discuss any potential side effects, and ensure the safety of medications provided. Once the patient is satisfied with the treatment outcomes, follow-ups will be scheduled every 3- 6 months for ongoing review.
